Onboarding

The four-week setup

Most practices are fully active within four weeks. There's nothing clinical for your team to do — just a short list of administrative items so records can start flowing into your EHR.

Week 1

Foundation

  • We introduce your team to your point of contact.
  • You start by uploading patient files to a shared drive — no EHR integration needed yet.
  • We agree on a simple weekly rhythm: which day you'll share your upcoming schedule.
Week 2

Patient Analysis

  • You share your patient roster (or the population you'd like prioritized).
  • Records are matched, reconciled, and prioritized by clinical complexity.
  • We confirm a delivery schedule with you before anything is sent.
Week 3

First Summaries Arrive

  • Your first batch of clinical summaries is uploaded directly to your EHR.
  • We confirm you've received them and answer any setup questions.
  • Nothing requires action outside your normal chart review workflow.
Week 4+

Ongoing Delivery

  • Each week, share your upcoming appointment schedule — a quick EHR export.
  • Our clinical team builds a summary for every scheduled patient, often ready before the visit.
  • That's the entire ongoing effort — no per-patient selection needed.
Why this sequence matters: Clinical summaries can only be built once files are received and your patient roster is matched — so the first two weeks are entirely setup, not delay. Once running, the ongoing effort is genuinely small: share your weekly schedule, and a summary gets built for every patient on it.

Optional Add-On — In Development

Turn each summary into a drafted CCM/RPM care plan

For practices running — or considering — a Chronic Care Management or Remote Patient Monitoring program, an add-on engine currently in development is designed to extend the clinical summary we already build each week into a structured care plan. It's an add-on to the Clinical Data program, not a separate service, and it's not a replacement for your CCM/RPM platform.

Every week, the Clinical Data program already delivers a clinician-reviewed summary to your EHR. That same summary is the input — no separate data collection, no second intake.

A care plan is drafted

Conditions, goals, interventions, and identified care gaps are drafted into a CCM/RPM care plan for provider review — not a blank page your coordinators have to build from scratch.

Delivered to your CCM/RPM platform

Once reviewed and signed off, the plan hands off into the Chronic Care Management or Remote Patient Monitoring tool your practice already runs. It's an add-on to the Clinical Data program, not a replacement for your CCM/RPM platform.
