Bring more care into the community.
Ellward helps pharmacies and physician practices participate in programs that expand community-based care, support membership services, and connect patients with the care teams they choose.
In partnership with MDHIN
For Pharmacies
Join the MDHIN pharmacy network
Independent and group pharmacies can join the MDHIN network to offer membership programs, point-of-care testing, and a branded way for patients to enroll.
There is no cost to join the MDHIN pharmacy network.
Available membership programs give patients a monthly relationship with your pharmacy.
Eligible point-of-care testing and test-to-treat services, where available in your state.
Provider support and escalation pathways are available where applicable to the services you offer.
A branded patient enrollment page where your patients can learn about and enroll in your programs.
Applicable claims are submitted to MDHIN using BIN/PCN — the claims workflow you already use.
Services vary by program, pharmacy, and state. Free network participation does not mean every service, piece of equipment, or implementation expense is free — some programs may involve costs specific to the services a pharmacy chooses to offer.
For Physicians
Collaborate with community pharmacies
Physician practices can participate in the MDHIN network to extend care into the community and give patients a consistent, coordinated point of access.
Collaborate with participating community pharmacies so patients have another point of access between visits.
Available physician membership programs give patients a predictable, transparent way to access care.
Provider involvement and escalation pathways apply where the program calls for physician oversight.
Applicable claims submission and payment processes run through MDHIN, using billing formats your practice already works with.
MDHIN is not insurance. Patients retain their choice of pharmacy. Program terms and availability vary.
MyMDHub
Patients organize and share their own records
MyMDHub is the platform patients use to connect available Medicare records, organize personal health information and uploaded documents, and share selected information with the pharmacists and providers they choose. MyMDHub is not an Ellward-branded product.
Patient-authorized connection
Patients connect their Medicare records to MyMDHub through Medicare.gov and control that connection at all times.
Kept up to date
With patient authorization, MyMDHub keeps connected Medicare records updated as new information becomes available, and lets patients share those records with the care teams they choose. New encounters and claims typically reach CMS on their own timeline, so an update reflects what's available from CMS as of that refresh — not necessarily every visit in real time.
Patient-controlled sharing
Patients choose exactly which records to share, with whom, and for how long. A sharing link's expiration is separate from the ongoing Medicare authorization, which patients manage on their own schedule at Medicare.gov.
Personal Health Hub
Illustrative demonstration — synthetic data, not a real patient record.
Medicare authorization renews on its own cycle and is managed by the patient at Medicare.gov, separate from any individual sharing link's expiration. Patients are never required to authorize record sharing before receiving unrelated services.
MyMDHub organizes and shares patient-controlled records. For how Ellward supports clinical information review and coordination for practices, see Clinical Data & Care Coordination.
